Obtaining Patient-Centered Medical Home Recognition: A How-To Manual

The Primary Care Development Corporation (PCDC) has developed a how-to manual for primary care practices that provides a comprehensive framework for navigating the NCQA Physicians Practice Connections-Patient Centered Medical Home (PPC-PCMH) recognition process.
