Brooklyn,
New York . In delivering this project, PCDC worked with a group of 11 healthcare providers, two Medicaid managed care plans, and dozens of community organizations representing underserved communities in
Brooklyn .
Goal
The Alliance aims to improve access to health services for women who require prenatal care and for children with asthma.
The
Alliance seeks to decrease racial and ethnic disparities among women and children in
Brooklyn so that all patients have access to quality healthcare. The
Alliance also uses technology such as the Internet to support participating teams.
PCDC Solution

Bring together healthcare providers and Medicaid-managed plans for a structured learning collaborative based on the IHI learning collaborative model.

Mobilize community resources to improve the health of the target population.

Share the best available scientific knowledge regarding the care of women needing prenatal care and children with asthma.
