NEW YORK CITY DEPARTMENT OF HEALTH AND MENTAL HYGIENE DIABETES AND DEPRESSION COLLABORATIVE
Goal
The New York City Department of Health and Mental Hygiene (NYCDOHMH) wanted to bring about widespread changes in diabetes and depression care by addressing clinical outcomes for persons with both diabetes and depression. The collaborative, which includes about 35 participating provider sites, also seeks to reduce racial disparities in underserved communities.
