WORKING WITH PCDC: EMERGING INITIATIVES
In addition to its capital financing and operations programs, PCDC offers expertise in designing, planning and implementing a wide variety of programs intended to improve performance and the quality of primary care services provided to underserved populations.
PCDC recognizes that the Learning Collaborative Model can be applied in many ways and that organizations with differing levels of expertise in improvement work may need different levels of technical support. PCDC's experience ranges from the total design of a clinical improvement collaborative, from selection of a clinical topic to the final outcomes congress, to targeted assistance in any of the discrete areas outlined above.
Using the framework of the Collaborative Learning Model, PCDC has designed a range of technical support services targeted to the essential components of any clinical improvement effort. These services, which can be used individually or as a package of services, include:
- Project Planning and Design. PCDC assists its clients in project design and planning services to meet their needs and those of their stakeholders and target audiences. This includes planning, group facilitation, proposal development and development and implementation of detailed workplans for project management.
- Stakeholder Coordination. By virtue of its mission, PCDC is uniquely positioned to bring together organizations and interested parties to achieve success with programs. Leveraging its goal to improve access, PCDC forges new relationships with and among healthcare organizations that often have no prior histories of working together. PCDC liaises with and convenes healthcare providers, health plans, research entities, consumer groups and community organizations to create previously unexplored opportunities to design and deliver programs that increase access, improve the quality of healthcare and ultimately result in healthier populations and communities.
- Capacity for Replication. PCDC’s integrates the core building blocks of the Collaborative Learning Model to sustain and spread improvement. This includes specific transition activities in addition to a step-by-step method for building capacity for long-term change.
- Data and Measurement. Services include database design, development of clinical patient registries and other clinically-oriented data collection and decision support systems, definition of relevant process and outcome measures for a particular clinical condition and data analysis and results presentation.
- Program Evaluation. Research-oriented staff schooled in the published literature on the Collaborative Learning Model will assess the structure and the process of your improvement effort and consult on possible modifications to your plans. PCDC staff can also evaluate improvement collaborative outcomes, which is often required by governmental and other funding agencies.
- Leadership Development. The part played by an organization's leaders is essential to the success of any clinical improvement effort. PCDC has defined a model of appropriate leadership roles and provides direct, hands-on support to leaders as they assess their current capability and make plans for strengthening their and their organizations' ability to make measurable improvements in clinical care.
- Team Coaching. PCDC offers coaches skilled in supporting front-line teams in rapid-cycle improvement work. Many organizations find that clinical improvement takes place more readily with experienced hands supporting their clinical and administrative staffs as they learn how to make effective change in clinical care.
